Web-based Assessment for Determining your Eligibility to Participate in our Study About Treatment for Anxiety and Anxiety Related Disorders
The Institute for Mental Health Research at the University of Texas at Austin and the Translational Reseach Program at Boston University are conducting an in-person treatment study investigating the effects of exposure-based therapy on Anxiety and Anxiety-Related disorders. Completion of the following web screening survey will take approximately 30 minutes and is the first step in determining your eligibility. You may complete this survey in multiple sittings. If we notice you haven't returned to complete this survey within one week, we will follow up to see if you remain interested in determining your eligibility.
Additional information about the study is also provided as part of this survey to inform your decision about whether you would like to participate. You may be compensated up to $300 for your full participation in the study. The study will consist of 14 weekly sessions as well as two follow-up interviews. There will be no monetary compensation for completing the steps in determining your eligibility. Note that being eligible for this study is a 2-step process. Completing this online survey is only the first step in evaluating your eligibility for the study. The additional steps will be described to you later in this survey.

Study Description
The main purpose of this study is to assess physical and emotional reactivity to two breathing exercises completed prior to a 12-week course of cognitive-behavioral therapy (CBT), and to determine how such reactivity relates to treatment outcomes in individuals with several forms of anxiety-related problems. The 12 sessions of exposure-based CBT are offered free of charge.

Taking part in this study consists of the following procedures:
(1) Online Prescreen: This prescreen involves completing several questionnaires about your anxiety-related symptoms, and how this impacts your lifestyle. If the study appears to be a good fit for you based on your responses, you will be invited to an in-person screening visit. This online prescreen takes about 30 minutes to complete.
(2) In-Person Screening Visit: At the in-person screening visit, you be asked to complete an interview to assess your anxiety-related symptoms and how this impacts your functioning. This visit will take about 90 to 120 minutes to complete and will be used to determine if this study is a good fit for you.
(3) Labratory Visit: At laboratory visit , you will undergo the two breathing challenges. These include (a) taking repeated full breaths of a harmless mixture of either 20% CO2 or room air; and (b) repeated hyperventilation (inhaling and exhaling rapidly and completely). During these tasks, we will measure your emotional and physical reactions. You will also be asked to complete questionnaires during this visit. This visit will take about 90 minutes to complete.
(4) 12 Treatment Sessions (CBT): The treatment phase will involve attending 12 weekly one-hour sessions of CBT. Treatment consists of 12 hours of CBT in total.
(5) Weekly Online Questionnaires and Assessment: Prior to every treatment session (CBT), you will be asked to complete online questionnaires (15-30 minutes) and an online interview (approximately 15 minutes) about your anxiety-related experiences.
(6) Online Post-treatment Questionnaires and Assessment: At one-week post-treatment, you will be asked to complete online questionnaires (15-30 minutes) and an online interview assessment (15 mintues) to assess your level of improvement with treatment.
(7) Online Three Month Follow Up: At a 3-month follow-up visit, you will complete the same online questionnaires and an online interview to determine how well you maintained your improvement following treatment. This visit will take 30-60 minutes to complete.

Determining your Eligibility
There are two steps in determining your eligibility:
First, you will complete this survey. This should take approximately 20-30 minutes.
Second, depending on your responses to this survey, you may be invited to complete an in-person screening interview. You will be required to bring any medications you are currently taking to this interview. I f you are eligible after this visit, you will be invited to take part in the study.
I understand that I will have to complete a in-person screening visit to further determine my eligibility and that this visit will take approximately 90 to 120 minutes to complete. I also understand that if I am currently taking any prescribed medications, that I must bring them all to this first visit.
